Lisa Hansen-Yarussi Western Washington Annual Scholarship
In honor of Lisa Hansen-Yarussi who fought bravely a 15 year battle with a rare neurodegenerative brain disease we are honored to offer the Lisa Hansen-Yarussi Western Washington Annual Scholarship this year.  An undergraduate student or students or students studying in Washington with Brain Injury will be awarded a scholarship in memory of Lisa’s life. This is a one-time only scholarship in the amount of $1,000 for a full time student or students (pro-rated for part time students based on credit load) paid to the institution on behalf of the students after acceptance into accredited vocational, technical school, college or university.Eastern Washington Student Scholarship
Each year a full time student, or part time students, studying in Eastern Washington will be selected to receive this scholarship. This is a one-time only scholarship in the amount of $1,000 for a full time student or students (pro-rated for part time students based on credit load) paid to the institution on behalf of the students after acceptance into accredited vocational, technical school, college or university.

Higher Education/Professional Scholarship
Each year a professional or student in postsecondary education who is committed to advances in Brain Injury knowledge in the state of Washington will be selected to receive this scholarship. This is a one-time only scholarship in the amount of $1,000 paid to either a professional or a student who will be enrolled in postsecondary education for the designated scholarship year who is committed to advances in Brain Injury knowledge (throughout Washington state).

Eligibility:
All applicants must live in Washington State
• Applicants to the “Lisa Hansen-Yarussi Western Washington Annual Scholarship” and the “Eastern Washington Student Scholarship” must
□ Be survivors of Brain Injury. Verification can be in the form of medical records or a vouching from the BIAWA staff, board members, or support group facilitator;
□ Be seniors in high school ready for graduation in the year the scholarship will be awarded or already enrolled in an undergraduate program;
□ Be enrolled full or part time for the academic year of award: award will be prorated if part time.
• All applicants to the “Higher Education/Professional Scholarship” must
□ Be students in postsecondary education, enrolled full time, who are committed to advances in Brain Injury knowledge;

Deadline – June 30th, 2016:
Applicants must submit an application with attachments postmarked no later than June 30th, 2016 for consideration for the 2016-2017 academic year. Scholarship recipients will be notified within 60 days of this deadline. Only the recipients will be notified. Incomplete applications will not be considered.Attachments Check List:
□ Verification of brain injury for applicants to the “Lisa Hansen-Yarussi Western Washington Annual Scholarship” and the “Eastern Washington Student Scholarship” (Verification of brain injury can be in the form of medical records or a vouching from the BIAWA staff, board members, or support group facilitator).
□ Verification of Brain Injury as area of educational interest for “Higher Education/Professional Scholarship”
□ Proof of acceptance/enrollment
□ Applicant’s most recent academic transcript □ One letter of recommendation from a colleague, educator or supervisor

Scholarship deadline is June 30th 2016. This is a one-time scholarship. Recipient will not receive multiple awards. Scholarship will be awarded on the basis of funds available and applicant eligibility. The BIAWA will not discriminate regardless of ability, gender, sexual orientation, ethnicity, or religion. Recipients are selected through an application process. Recipients may only use the scholarships to attend an accredited vocational, technical school, college or university. All scholarships are paid to the institution on behalf of the student and are paid in full for the fall quarter.

BIAWA reserves the right to distribute The Scholarship between multiple eligible applicants.
